DS-11 Passport Application
* Complete and print 2 copies (Single-Sided Only) of the Online Government Passport Application Form DS-11.

Passport Photo
* Color photo must be 2 x 2 inches and have a white background and be professionally taken within the past 6 months.

Proof of U.S. Citizenship
* Must be an ORIGINAL or CERTIFIED copy with a raised or embossed seal of an U.S. Birth Certificate (with both parents names on it).

Government Fees
* The amount of the check will be either $190 for the Passport Book only or $220 for a Book and Card. Make the check payable to the "US Department of State".
* A pre-printed Name and Address must be located in the upper left hand corner.
